EMS and Microcurrent
EMS and microcurrent are often sold together, but the labels are not synonyms. EMS, also called NMES in research, uses electrical pulses to activate motor nerves and produce a muscle response. "Microcurrent" usually signals lower-level current, yet listings do not always state the output. Compare waveform, current range, electrodes, intensity settings and intended area.
A 2012 randomised controlled trial studied a specific NMES facial device in 108 healthy women over 12 weeks. Participants assigned to the device used it for 20 minutes a day, five days a week. Assessor-blinded ultrasound measurements showed an 18.6% mean increase in zygomatic major muscle thickness from baseline in the NMES group, while the non-intervention control group did not show the same change. The trial was only partially blinded, and subjective appearance ratings are vulnerable to expectation. It supports that device and protocol, not a universal "18.6% lift" claim.
A newer eight-week split-face study of facial NMES involved 24 women and reported changes in elasticity, wrinkles, jawline angle and blood flow on the treated side. The sample was small, the frequency range was device-specific, company researchers were among the authors, and the article later received a correction.
The 2024 review Development of Home Beauty Devices for Facial Rejuvenation identified 18 relevant studies, only five of them randomised controlled trials, and just one trial in its microcurrent section. Small samples, short follow-up and mixed designs limit broad comparisons.
What to check on an electrical facial device

Mechanical Massage

In a 2018 study, a five-minute roller session on one cheek increased local skin blood flow for at least 10 minutes in 12 participants. A five-week phase with 14 participants found a change in vascular response to heat. These were blood-flow endpoints, not proof of permanent lift, and one author was affiliated with the manufacturer.
A 2025 randomised trial compared roller and gua sha routines in 34 women over eight weeks. Both groups changed on selected measurements, but there was no untreated control. The study cannot separate the routine's effect from natural variation or measurement effects.

LED Photobiomodulation
LED devices deliver light to skin. They do not train facial muscles, even when an LED wand is sold as a toning device. An immediate contour change after using a handheld tool may reflect pressure, products, expression or photography, not light mechanically lifting tissue.
A 2025 multi-centre, double-blind, sham-controlled trial studied a 630 nm red and 850 nm near-infrared home mask in 60 adults for 16 weeks. It found between-group differences in crow's-feet grading. The result belongs to that mask, output and eye-area endpoint, not every coloured LED mode.
The American Academy of Dermatology says results vary and long-term effects need more study. It advises following directions, using specified eye protection, and consulting a dermatologist about suitability. Its guidance also notes visible-light sensitivity and hyperpigmentation concerns for darker skin tones.

Non-Invasive Radiofrequency
Non-invasive surface RF creates controlled heating in tissue. It targets skin rather than training muscle like NMES. Temperature, movement, contact and safety controls all matter.
A 2022 randomised split-face trial evaluated a home device combining bipolar RF, 630 nm LED and gel in 33 women for 12 weeks. Several skin outcomes changed versus the cosmetic-treated side. The study cannot isolate RF from LED and gel, participants knew the device side, and the supplier funded the publication fee.
Keep that result tied to the tested design, temperature controls, gel, schedule and population.
Surface RF and RF microneedling are also different categories. RF microneedling uses needles to deliver energy below the skin and is not an at-home facial toning method. The US Food and Drug Administration's 2025 RF microneedling safety communication described reports including burns, scarring, fat loss and nerve damage, and stated that RF microneedling is a medical procedure that should not be performed at home.
